School Nurse
Rowland Hall has a School Nurse!
WHEN TO CONTACT THE SCHOOL NURSE:
Any medical conditions, both acute and chronic, that effects your student’s school day.
Any medications, over the counter or prescription, that needs to be administered during school hours. See Medication Administration form.
When your child has a communicable illness, i.e.: strep throat, mono, influenza, etc.
Shirley Stevens RN, BSN (Nurse Shirley) is the school’s part time school nurse.
School nursing is a specialized practice of professional nursing that advances the well-being, academic success and life-long achievement of students.
The school nurse provides direct health and care to students and staff, screening and referral for health conditions, promotes health and a healthy environment, develops health policies and programs, and serves as a liaison between school personnel, family, community and health care providers.
The school nurse is responsible for medication administration and development of an individualized nursing care plan including an emergency action plan for students and staff with medical needs.

MEDICAL INFORMATION AND FORMS REQUIRED
IMMUNIZATION FORMS as required by Utah State law. All students must have current immunization form on file prior to the first day of school. See division requirements.
MEDICATION ADMINISTRATION form-to be completed and signed by a health care provider each school year and returned to school.
